Longevity Life Advisors partners with Laboratories, Health Advisory Services and other healthcare providers, employers, insurers and other entities to provide services to members who purchase a membership program. We are not an insurance company, and we do not provide insurance products or services. Longevity Life Advisors is a company that offers membership services that provide disease information through access to multiomic services and counseling provided by licensed clinical laboratories and physicians, but we are not a medical provider. Longevity Life Advisors does NOT provide medical treatment, or health care services of any type, makes no medical referrals, issues no prescriptions, is not an insurance product or service. You should consult with medical professionals regarding all treatment decisions for any medical condition you may have.
1. Information We Collect

You can generally visit the Site without revealing any personal information about yourself. However, to access the Services we may ask you to provide certain personal information and without providing such personal information, you may be unable to access the Services. The following is a list of information that we may collect from or about you:

In addition, when you request Services from Us or the Laboratories, Advisors, Practices or Providers through your membership, you may share:
· Personal information: your name and contact information and demographic information PHI (Defined below), including medical and family history submitted for genetic testing and counseling purposes, as well as current clinical diagnoses or other reason for visiting a Provider
· Date of visit
· Images or videos you share for counseling purposes
· Communications with the Practice or its Providers
The information you provide to a Laboratory, Practice or Provider through your membership or in connection with the Services may be protected under applicable federal and state laws applicable to health and disease information, including but not limited to the Health Insurance Portability and Accountability Act of 1996, Public Law 104-191, and its implementing regulations, as may be amended from time to time (collectively, “HIPAA”). Longevity Life Advisors partners with Laboratories, Health Advisory Services and other healthcare providers.
We may collect Personal Data from individuals that interact with Longevity Life Advisors for a variety of reasons, including, but not limited to users of our online services and websites, patients and users of our products and services, clinical study candidates and participants, healthcare providers, clinical investigators and members of investigator teams, employee and apprenticeship candidates, customers and business partners and their employees, agents, and contractors, Attendees and Participants at Events, Customers Who Purchase Directly from our Websites. The legal bases for processing may include:

· in preparation for or to perform a contract with you
· in circumstances where we have requested and received consent and for other purposes that may be required or allowed by law

· Membership Services: To provide DNA genomic screening, precision health services, and related wellness services.
· Payment: To process membership fees and service charges.

· Research: To use de-identified, population-level data for research or commercial purposes.
3. Information Sharing & Disclosure
We disclose PHI only as permitted under HIPAA:
· With Authorization: Shared only with your written consent.
· Without Authorization: As permitted by HIPAA for treatment, payment, healthcare operations, and legal requirements.
We will not sell, rent, or commercialize your PHI without your consent.

We use administrative, physical, and technical safeguards to protect your PHI, including:

5. Your HIPAA Rights

· GINA: No use of genetic information for insurance or employment decisions.
· ADA: No discrimination based on disability.
· State Laws: Compliance with CCPA, CPRA, and other applicable laws.
7. Cookies & Tracking Technologies
We may use cookies and tracking technologies for analytics or marketing in compliance with HIPAA and privacy laws. PHI will not be compromised.
8. Changes to this Policy
We may update this Privacy Policy as required by law. Updates will be posted with a revised effective date.
